Supported Living & Outreach Support – Process and Value

Our Approach

We deliver person-centred supported living and outreach services that enable adults with learning difficulties to live independently, safely, and with dignity in their own homes and communities.

Our model is structured, consistent, and outcome-focused, ensuring each individual receives the right level of support at the right time.

Process:

  1. Referrals received from local authorities, families, or professionals
  2. Initial discussion to understand needs, risks, preferences, and goals
  3. Review of existing assessments and care plans.
